Most advice about under-eye bags fails for one reason. It treats two completely different problems as if they were the same problem.
Puffiness is fluid. It arrives overnight, it responds to cold and drainage, and it is usually gone by lunchtime. A true under-eye bag is structural. It is there when you wake up, it is there at midnight, and no amount of cold spoons will move it.
Knowing which one you have is the whole thing. Here is how to tell, and what genuinely works for each.
Under-eye bags or puffiness? How to tell
Check twice in one day, in natural light. Once within ten minutes of waking, once in the late afternoon.
- Noticeably better by the afternoon means fluid. This is puffiness, and it is highly responsive to treatment.
- Unchanged all day means structure. This is a true bag, and it needs a different conversation.
- Both is the most common answer. A structural bag that fluid makes worse on bad days.
One more check. Tilt your chin up towards the light. If the shape flattens out, some of what you are seeing is a shadow rather than a bulge, which points towards dark circles and hollowing instead.
What to reach for
Before the detail, the short answer for each.
- Puffy in the morning, better by lunch. De-Puff Eye Treatment.
- Puffiness plus fine lines and crepiness. Supreme Eye Serum.
- There all day, and you want it better today. Instant Fix Eye Lift Gel, with Firming Eye Cream twice daily.
Good news for most people reading this: the majority of what gets called a bag is fluid, and fluid is the version that responds quickly.
What causes puffiness
Fluid gathers in the space below the eye and has nowhere to go. The skin there is the thinnest on the body, so a volume of water that would be invisible on your arm is obvious on your face.
- Sodium. A salty dinner shows under the eyes the next morning more reliably than anywhere else.
- Alcohol, caffeine and dehydration. All three prompt the body to retain water. Dehydration is the counterintuitive one. Drink less, hold more.
- Sleep, and sleeping position. Lying flat or face down lets fluid pool overnight. Poor sleep compounds it.
- Dry air and altitude. Low humidity shifts fluid in the eye area. Worth knowing before a flight or a ski trip.
- Allergies and sinus congestion. Both cause swelling and rubbing, and rubbing makes everything worse.
- Crying, and the hours after it. Self-explanatory, and it passes.
What causes true under-eye bags
This is anatomy rather than lifestyle, which is why it does not respond to the same tactics.
The fat that cushions the eye sits held in place by a thin membrane called the orbital septum. Over time that membrane weakens and the fat drifts forward into the area below the eye, where it creates a visible bulge. At the same time the skin above it thins and loses collagen, so there is less holding anything back.
Genetics and eye socket anatomy determine a great deal of it. Some people develop bags in their twenties with a faultless routine, and no product is going to change that inheritance. Sun exposure accelerates the collagen loss underneath. Smoking accelerates all of it.
Dr Sebagh Top Tip'There are no sebum glands in the eye socket area, so there is no oily protection barrier. The skin here is fragile and it is the first to show what is happening underneath. Use a good eye cream from an early age.'
What actually works for puffiness
Fluid responds to two things: cold, and drainage. Everything below is a way of delivering one or both.
The morning routine
- Keep your eye treatment in the fridge. Cold constricts the blood vessels and helps move fluid out of the tissue. This is doing most of the work.
- Apply it along the orbital bone, working from the inner corner outwards. Tap, never drag.
- Water before coffee. And go easy on salt the night before anything that matters.
Choosing your eye treatment
Each of these is a complete treatment in its own right. Choose the one that matches your concern rather than stacking them.
| Eye Treatment | Best For | Format | Use |
|---|---|---|---|
| De-Puff Eye Treatment | Morning puffiness, fluid retention, tired eyes | Cooling roller ball | AM, kept in the fridge |
| Supreme Eye Serum | Puffiness with fine lines, crepiness, loss of firmness | Cooling roller ball | AM and PM |
| Instant Fix Eye Lift Gel | An immediate lift, tired-looking eyes, wear under make-up | Lightweight gel | AM, or before an event |
| Firming Eye Cream | Dryness, fine lines, thin or fragile skin | Fragrance-free cream | AM and PM |
De-Puff Eye Treatment is the one for fluid. It targets the three drivers at once: sluggish microcirculation, poor drainage of water and waste, and accumulated fatty material. Palmaria Palmata (red seaweed) extract supports the elimination of waste, M2NC Complex reinforces firmness and tone, and soya lecithin, a soybean-derived blend of phospholipids, works alongside Zea Mays to support circulation in the area. Its cooling roller ball adds gentle drainage to the cold.
Supreme Eye Serum is the comprehensive treatment for the eye area. Hyaluronic acid, hydrolysed sesame protein, an LDH Complex of three active molecules and multiple peptides work to illuminate, tighten and refresh while reducing the appearance of fine lines and deeper wrinkles. M2NC Complex reinforces firmness and tone and supports the elimination of the waste material behind puffiness and dark circles. It also has a cooling roller ball. If you want one treatment doing the most across puffiness, texture and firmness, this is it.
Instant Fix Eye Lift Gel is the immediate one. Biotech ingredients and hyaluronic acid give a visible lifting effect on application, and it sits well under make-up. For a morning that needs to look better than it was.
Firming Eye Cream is the twice-daily base, and it is 100% fragrance-free. Well-hydrated, firmer skin holds the area better whichever treatment you use alongside it.
Most people start with one and add the second once they see what it does.
In the clinic, in half an hour
The Ultra Eye Lift Facial is a 30 minute treatment at the Dr Sebagh Clinic built around exactly this problem. It uses Ultralift HIFU technology with two heads made specifically for the clinic, working under the eye to drain puffy fluid and along the sides to soften crow's feet, followed by massage to improve circulation around the eye. A tailored eye serum and eye cream finish it.
If you want the fastest visible change before something that matters, this is it, and you can book it online.
The habits that change it
- Sleep on your back with your head slightly elevated. Avoid sleeping on your front.
- Reduce sodium, particularly in processed food, where most of it hides.
- Hydrate consistently rather than in bursts.
- Treat allergies properly rather than rubbing your eyes.
- Take SPF right up to the lash line, and wear sunglasses that block UV.
What works for structural bags, honestly
Here is the part most brands skip.
Skincare has a real job here, and it is worth being precise about what it is. Firmer, better-hydrated skin over a bag makes the whole area look smoother, brighter and more rested, and it slows the laxity that makes a bag more pronounced over time. That is a visible difference, and Firming Eye Cream twice daily is the way to get it. Instant Fix Eye Lift Gel does the same job on a shorter timeline, with a lifting effect you can see on application.
What no cream does is remove a displaced fat pad. Anything promising that is overpromising. Where the structure itself is the problem, the options are in-clinic, and the most useful of them work on the same principle: improve the quality of the skin rather than add to it.
Polynucleotides
Polynucleotides are purified fragments of DNA and RNA building blocks, formulated as an injectable gel. They work differently from filler. Rather than adding volume, they act as a biological signal that supports repair and regeneration in the tissue itself, improving hydration and elasticity, supporting the fibroblast activity behind a healthier collagen and elastin environment, and calming inflammation, which suits fragile or reactive skin.
Under the eye, where the skin is at its thinnest, that distinction is the point. Many patients do not need volume. They need better texture, better resilience, less crepiness and a fresher, brighter look. Polynucleotides improve the quality of the skin there without heaviness, softening fine lines and crepey texture for a more rested appearance.
Results are progressive. Hydration and smoothness often improve within a couple of weeks, but the real change builds over six to twelve weeks. Treatment is done as a course, usually two to three sessions spaced a few weeks apart, with maintenance from there depending on the skin.
Radiofrequency with microneedling
RF with microneedling delivers micro-fractional bipolar radiofrequency energy alongside skin needling to tighten skin and improve texture and tone. Because collagen takes time to build, the tightening develops gradually over around six months. Usually three to four sessions, spaced one to three months apart.
The Ultralift Eye Treatment
The Ultralift Eye Treatment was pioneered at the Dr Sebagh Clinic. Warm pulses of ultrasound energy are delivered deep under the skin, causing the muscle layer to contract, while the gentle heat encourages the skin to produce fresh collagen of its own. The area becomes firmer and more elastic, and tightens further as that new collagen builds.
Dr Sebagh commissioned a bespoke head attachment shaped specifically to reach the eye area, which allows the team to tighten the loose orbicularis oculi muscle across the upper and lower lid, and to lift the brow.
The technology is offered two ways. The Ultra Eye Lift Facial is the 30 minute facial version, delivered by a facialist, and you can book it online. The Ultralift Eye Treatment is the doctor-led treatment, booked with the clinic by phone or email rather than online. Our clinic enquiries page has the details.
Worth being precise about what this does for a bag. It tightens the muscle and the skin around the eye, so laxity improves and the contour looks firmer and more rested. It does not remove a displaced fat pad. The clinic often combines it with RF microneedling to address crepiness in the area, and with PRP or Botox.
A note on filler under the eye
Dr Sebagh does not inject hyaluronic acid into the eye socket itself. Lymphatic drainage is slower in this area, so filler placed here can create puffiness or visible product in the wrong patient, which is the opposite of what someone treating under-eye bags is looking for.
Where a bag is exaggerated by hollowing beneath it, he rebuilds the contour around the eye instead, treating the deep fat compartment beneath the eye and the surrounding malar and zygomatic areas. Nano-fat injections are another route to improving skin quality in the area.
When surgery is the honest answer
Where a bag is entirely structural and someone wants it removed rather than improved, the treatment is lower eyelid surgery. The Dr Sebagh Clinic does not perform it, and will refer you rather than offer you something that will not do what you are asking for.
A consultation is the only way to establish which of these applies to your anatomy. Treatments with the doctors are arranged by phone or email through our clinic enquiries page, not booked online.
What does not work
- Cucumber slices and cold tea bags. The cold helps. The cucumber and the tea are contributing nothing the fridge was not already doing.
- Haemorrhoid cream. A genuinely bad idea near the eye. It constricts vessels briefly and irritates very thin skin considerably.
- Spoons, ice cubes applied directly, anything frozen solid. Same principle as the fridge, more risk to the skin. Use a cooled product instead.
- Face taping and overnight patches, for structural bags. Temporary at best on fluid, irrelevant to anatomy.
- Rubbing and massaging aggressively. The skin here has no cushioning. Pressure makes the laxity worse over time.
Frequently asked questions
What is the difference between under-eye bags and puffiness?
Puffiness is fluid retention. It fluctuates through the day, is usually worst in the morning, and responds well to cold, drainage and lifestyle changes. Under-eye bags are structural, caused by fat drifting forward as the orbital septum weakens. They stay constant through the day and do not respond to the same measures.
Can eye cream get rid of under-eye bags?
It can visibly reduce fluid puffiness. It cannot remove a structural bag, because the cause sits beneath the skin rather than in it. What it does do is improve hydration, firmness and texture in the skin above, which makes the area look smoother and better rested.
Do polynucleotides help with under-eye bags?
Not directly. They do not add or remove volume, so they will not shift a displaced fat pad or drain fluid. What they do is improve the quality of the skin over the area, softening fine lines and crepey texture for a fresher, more rested look. For many people that visible improvement is what they were actually after.
Is there a treatment that works on puffiness quickly?
The Ultra Eye Lift Facial is a 30 minute treatment using Ultralift HIFU technology, working under the eye to drain fluid and lift the eye contour, with massage to improve circulation. It is the fastest visible option for the fluid side of the problem.
Is surgery the only way to remove under-eye bags completely?
For a purely structural bag, yes. Removing a displaced fat pad is lower eyelid surgery. The Dr Sebagh Clinic does not perform it and will refer you if that is what you need. Everything short of surgery improves how the area looks rather than removing the bag itself, which for many people is enough.
Why are my under-eye bags worse in the morning?
Fluid pools overnight while you are lying flat, and there is nowhere for it to go in the thin skin below the eye. If the bag improves within a couple of hours of getting up, it is largely fluid.
Do under-eye bags go away on their own?
Fluid puffiness does, usually within hours. Structural bags do not, and they tend to become more pronounced over time as the septum weakens further and the skin loses collagen.
At what age do under-eye bags start?
Most commonly from the mid-thirties onwards, but genetics override age entirely. Plenty of people develop them in their twenties, and it is not a reflection of their routine.
Does drinking more water reduce under-eye bags?
It reduces the fluid component. Dehydration prompts the body to retain water, so consistent hydration alongside lower sodium usually shows in the eye area within a day or two. It has no effect on a structural bag.
Can under-eye bags be a sign of something medical?
Usually not. They are most often age-related or genetic. Sudden, severe, painful or one-sided swelling is worth raising with a doctor rather than a skincare brand.
Where to start
If the shape changes through the day, it is fluid, and De-Puff Eye Treatment is the fastest thing you can do about it. If it does not change, Firming Eye Cream and Instant Fix Eye Lift Gel will visibly improve the skin over it while you decide whether you want to go further.
